I have used this text when preparing teaching sessions and recommend it to my students. It is comprehensive, detailed but also quite readable. There are some aspects which as an american text are not applicable in the UK, e.g. around billing and codes but this does not detract from the value of the text. There is such a wealth of information and it is so well referenced and supported, this will remain a key text for me for some time.
